Patient Registration
Surgical Patients
A staff member will communicate your arrival time to complete registration paperwork prior to your surgery. To ensure timely registration, please bring the following:
- Insurance ID card(s)
- Driver's license or personal ID card
- Any paperwork from your physician, such as test results
- Prescription medications in original containers, and a list of over-the-counter drugs, nutritional supplements, and herbal supplements, including all original containers of your prescription medications. Pill boxes are not acceptable.
- Payment for your insurance co-pay
Please remember to follow any specific preparation and/or dietary restrictions communicated by your physician's office at the time of scheduling. Failure to do so may result in the rescheduling of your appointment.

Lab and Imaging Patients
A staff member will communicate your arrival time to complete registration paperwork prior to your procedure. To ensure timely registration, please bring the following documentation:
- Insurance ID card(s)
- Driver's license or state-issue photo ID card
- Physician order forms
- Payment for your insurance co-pay, if any
Please remember to follow any specific preparation and/or dietary restrictions communicated by your physician's office at the time of scheduling. Failure to do so may result in the rescheduling of your appointment.
